Role Overview
We are seeking a highly motivated and experienced PCN Clinical Pharmacist to join our team on a full-time, remote basis. You will work collaboratively with GPs, nurses, and other healthcare professionals to provide expert medicines management, structured medication reviews (SMRs), and support long-term condition management. This role offers the flexibility of remote working while making a significant impact on patient care within PCNs.
Key Responsibilities
Conduct structured medication reviews (SMRs) for patients with polypharmacy, long-term conditions, or those at risk of medication-related issues.
Support medicines optimisation and ensure safe, evidence-based prescribing across the PCN.
Provide clinical advice and support to GPs, nurses, and other healthcare professionals on medication-related issues.
Assist in the management of repeat prescribing systems, medication reconciliation, and deprescribing where necessary.
Support patients with chronic disease management, including diabetes, cardiovascular disease, and respiratory conditions.
Review and action clinical queries related to medications, including hospital discharge summaries and outpatient prescriptions.
Contribute to antimicrobial stewardship and support best practice in prescribing antibiotics.
Provide medicines-related education and training to staff within the PCN.
Work within local and national guidelines, including NICE guidance, to ensure compliance and best practices.
Assist with audit, quality improvement projects, and performance monitoring to enhance medication safety and efficiency.
Essential Requirements
GPhC-registered pharmacist with the right to work in the UK.
Minimum 6 months of primary care or PCN experience
Excellent clinical decision-making and communication skills.
Ability to work independently in a remote setting while collaborating with multidisciplinary teams.
Desirable Qualifications & Skills
Experience in frailty, care home medication reviews, or mental health medication management.
Understanding of QOF, IIF, and DES targets relevant to PCNs.
Strong organisational and problem-solving skills.
